Price each month shows the amount the carrier quoted based on your benefit level, duration and health rating, payable in twelve equal installments. Classification indicates the underwriting tier used for that quote; your carrier confirms the final tier post-underwriting. Medical exam indicates if an exam is required, optional, or not needed. Company strength displays A.M. Best's financial ratings.
